45 year old man with severe left neck and shoulder pain that he had to quit his job. When distracted, his neck tilts to the left. The pain is local and spasmodic. There was focal muscle tension and tender but neuro-function is intact. Negative neck facet pain and facet joint loading test. What is the diagnosis and treatment option.

73 year old man with history of COPD was admitted to hospital for worsening symptoms. He suddenly  developed severe short of breath, gasping for air,  and severe left chest pain one morning during rounds. ​What is your diagnosis and how do you proceed next.

Case 133  Back pain

82 year old woman with 2 weeks of sudden onset, progressively worsening middle to lower back pain (without abdominal pain nor leg pain), fever, malaise, and oliguria. What is your differential diagnosis and plan. Outcome and ref withheld.

Case 134 Calf pain

68 year old lady with dull left calf pain with walking for 9 months.  Right leg is normal. She is obesity but not diabetic and she is fairly active with her daily activity. There is no weakness nor numbness but some focal tenderness as shown. What could be possible causes? Outcome and ref withheld.

The back pain shoots to her both legs with lightning sensation. Her left leg then became progressively weak.
51 year old lady with abdominal pain, back pain and leg pain...She could not maintain balance with her eye closed. Outcome and ref withheld. 

CASE 136 Neck PAIN

67 year old man with 25 year history of achy right side neck pain. The pain of achy and squeezing quality. He felt the pain at times to his right neck, shoulder, arm and forearm. The pain happens with turning his head to right, driving, working on his computer. Exercise and lifting did not trigger his pain. He had no other pain/symptoms/signs with non-focal neurological exam. The pain is shown above, the focal neck tenderness and provocative posture also shown. 

He had neck MRI and EMG/NCS done. These all support a mild to moderate degenerative disc diseases more right then left. Yet, cervical epidural steroid injection done by different doctors over several years had not helped at all. What could be a differential diagnosis? 

​CASE 137 headache

62 year old woman came to see her pain doctor for her lower back pain which had improved with physical therapy. Her doctor asked her via a review of system. She reluctantly told him she had a headache as shown but "not a migraine". The difference being that her current headache didn't go away in days like her regular migraine. On further inquiry, she said she had this headache for almost 2.5 weeks after a near fall in her kitchen to avoid stepping on her cat.  Her general and neurological examination is none focal. She was healthy otherwise. Please provide a different diagnosis and rationale for action.

71 year old lady with a 2 months history of calf pain. She was overweight and sedentary most of her time. She had some bilateral, symmetric, mild lower leg edema. What is diagnosis and plan?

​CASE 139 shin PAIN

16 year old avid tennis player had a 3 months history of shin pain as shown. She was healthy otherwise. She continued to play tennis and took some OTC pain medication for the pain. What is this pain?
